Our Services
At CRC, we deliver a broad range of assessment, treatment and consultancy services to clients of varied diagnoses all within the comfort of their own home. We believe that all clients in the community have the right to access high quality allied health services that are delivered on time. Our clients include but are not limited to; Home Care Packaged clients, DVA, NDIS participants and, private clients. Our services are delivered to clients in their homes to ensure that all aspects of daily living are assessed and has the added benefit that the client does not need to leave their home which is a great convenience for those that have mobility and transportation issues. These home visits ensure that the best clinical outcomes are achieved.
Home Assessment:
This service involves assessing a client within their own home, supported accommodation or residential care facility. Recommendations will be made related to equipment, treatment, support needs, and services to maintain safety and improve independence. A variety of daily living tasks may be reviewed as part of this assessment, including; personal care, domestic tasks, independent living skills and community activities (including transportation and recreation). Follow-up care, annual reviews and referrals to other services as needed
Home modifications:
We can assess a client in their own home and make home modification recommendations to improve access and safety. We can help design and coordinate minor home modifications including ramps, rails, non-slip strips, magnetic door holders, step mods and lever taps. We also work with funding bodies, builders regarding major home modifications including bathroom and kitchen modifications, major ramp structures, ceiling hoists and platform lifts.
Equipment Assessment:
There is a wide range of adaptive equipment to help improve safety and independence. We are skilled in assessing and prescribing this equipment which can include but limited to the following: personal care equipment, mobility aids including power and manual wheelchairs, scooters, hoists, pressure care equipment, personal alarm systems, seating, portable ramps, continence products, and many more. We keep abreast of the changing options of adaptive equipment and encourage all of our therapists have updated training with our continuing education program. We also work closely with suppliers to arrange trials and customisations.
Therapy:
We provide occupational therapy, physiotherapy and exercise physiology services to clients in their own home. We can work with clients over a period of time to help them to achieve goals related to personal care, domestic duties, community needs. We provide services to clients with a range of abilities including clients with complex needs. We can also provide specialised therapy services including falls and accident prevention, balance programs, upper limb rehabilitation, arthritis management, rehabilitation after hip and knee replacements, remedial massage, exercise programs for mobility and weight management, rehab after stroke, chest physio, exercises to improve continence and bone density, issues with pain, back care, energy conservation techniques and caregiver education.
Home Care Packages:
The Referral for clients with a Home Care Package service is designed to support individuals in maintaining a safe and independent lifestyle in their own homes. Clients who receive a Home Care Package can access a range of allied health services tailored to enhance their mobility, strength, and overall well-being. These services include:
- Occupational Therapy: Helps clients with daily tasks, home modifications, and adaptive strategies to make everyday activities easier and safer.
- Physiotherapy: Focuses on physical rehabilitation, pain management, and improving mobility.
- Exercise Physiology: Provides personalized exercise programs to increase strength, balance, and physical fitness. These services ensure clients can continue living at home with greater safety, comfort, and independence.
NDIS:
The National Disability Insurance Scheme (NDIS) is a national program that enables people with a disability to choose their own supports and services to meet their personal needs and goals.
CRC is a registered provider of occupational therapy and physiotherapy services to NDIS participants.
At CRC we offer high quality and tailored services to assist you to achieve your goals. We are community based and provide all our services at your home for your optimal assessment. Our therapists are highly skilled and can help you with your equipment needs, home modifications, vehicle and transport needs, coordination of support needs, therapy needs, exercise prescription and specialist assessments to determine functional capacity, independence/safety and assistive technology needs, goals related to independence.
Exercise Physiology
Exercise physiology is the study of how the body responds to physical activity and how it adapts over time. It focuses on various systems in the body—such as muscular, cardiovascular, respiratory, and metabolic systems—and examines their roles during exercise.
